What are the surgical options for treating fibroids?

BY DR. BROOKE WINNER

Surgical options for managing Fibroids include three main procedures: radiofrequency ablation (Acessa procedure), myomectomy, and hysterectomy.

  • Radiofrequency Ablation (Acessa): This minimally invasive technique involves using a probe to heat and destroy the fibroids. It’s done through small incisions, using a camera for guidance. While it can provide symptom relief and shrink fibroids by up to half, it may not eliminate all symptoms, and it is not currently recommended for women planning to become pregnant afterward.

  • Myomectomy: This procedure removes fibroids while preserving the uterus. It can be done in different ways depending on the fibroid’s location. If fibroids are mainly inside the uterine lining, they can be removed via hysteroscopy with minimal recovery. Larger fibroids in the wall may require laparoscopic or robotic surgery to extract them, leaving the uterus intact for potential future pregnancies.

  • Hysterectomy: This procedure involves the complete removal of the uterus and all fibroids. It can be performed vaginally, laparoscopically, or through a larger incision. Hysterectomies eliminate periods and prevent fibroids from returning. The fallopian tubes are often removed during this procedure to lower the long-term risk of ovarian cancer while keeping the ovaries intact.

Each option has its indications and benefits, so discussing with a healthcare provider is essential to determining the best choice for individual situations.
